Phyllis A. Leo, 94, of Forest City died at home Saturday. She was the widow of John J. Leo, who died in 2009.

Born in Carbondale, she was the daughter of the late Carmino and Mary Siranni Falbo. She was a graduate of Benjamin Franklin High School, Carbondale and was a homemaker. Phyllis was a member of Our Lady of Mt. Carmel Parish, Carbondale.

She was a loving mother and grandmother who will be missed. The family would like to thank Tina Bennett a special friend and caregiver.

She is survived by three daughters, Phyllis and husband Jan Crocker, Auburn, PA, Maryanne and husband Paul Debish, Forest City and Joan Stefanski, Slatington, four grandchildren, Paul, Erin, Rachel and Kaitlin and four great grandchildren, Paul, Hannah, Carter and Henry, a brother, Santo Falbo, Bethlehem, two sisters, Carmella Falbo, Forest City and Mary Carbo, Clearwater, Fl, nieces and nephews.

She was preceded in death by a son, John G. Leo.

The funeral will be Wednesday with Mass of Christian Burial at 11:00 AM in our Lady of Mt. Carmel Church, 15 Fallbrook St., Carbondale. Interment will be in Our Mother of Sorrows Cemetery, Greenfield Township. Friends may call Wednesday at the church from 10:00 AM until 11:00 AM.
